Linkage to Cure Coordinators (5)
Acadiana CARES, in conjunction with the Louisiana Office of Public Health-STD/HIV Program, is currently recruiting eight individuals (3 in New Orleans; 1 in Shreveport; and 1 in Lafayette) to provide testing follow up for persons suspected of having Hepatitis C (HCV) to confirm their diagnosis and educate them on the importance of curing HCV, to provide treatment adherence services for persons confirmed of having HCV and to connect them to medical care and community-based supportive services, and to provide specific medical information and navigation services to individuals with barriers to cure.
Excellent skills in community engagement, facilitation, quality assurance and policy and procedure development. General knowledge of Hepatitis C and/or STIs, case management, transmission, treatment adherence, health education and risk reduction.
Required: 6 years of experience in community engagement and working with vulnerable populations.
Preferred: A Bachelor’s degree in a social science or human related field with 2 years of experience in community engagement.
